You and the team will provide a short-term intervention to clinically review, and offer care and advice for patients when they are discharged from hospital. You also support a person during a period of ill health, and aim to avoid a hospital admission if a person’s care and medical needs can be met safely in their own home, including specialist care and support at home for end of life patients.

Therefore, as part of our UCR team you will receive extra training and support to develop the specialist skills required for this role. UCR Practice Educators are available to support with education, training and competency sign off, colleague development and career pathway support, and provide a link between teams for wider communication.

As a Band 3, you will have a Level 3 Diploma / NVQ 3 in Health and Social Care or equivalent and ideally have experience of working with older people in a community setting. You will also need to be competent with IT as electronic recording is an essential part of the role.
